This demo allows the you as a customer to create a customer profile which can then be edited and maintained. Simply fill out the form and click the button to save your profile. If you have filled out the form correctly your saved profile will be displayed. You can then click the button to change any of the information in your profile.

Although this demo uses a health profile as an example, the customer profile application can be applied to a any type of online business.

For the purposes of this demonstration, any profile information you create will only persist until you close your browser. Also, in order for this demo to work properly, you must have cookies enabled in your browser.
